Family Handbook Acknowledgement
Required upon enrollment. After reading the Family Handbook, please sign and return this form to your center director/site supervisor along with your other enrollment forms.
Parent Information About Inhaled Medication Procedures

Inhaled Medication Authorization
Required upon enrollment if your child requires inhaled medication.
Epinephrine Authorization
Required upon enrollment if your child requires an EpiPen.
